To complete the form(s):

• Open the applable registration form and please type in all requested information. NOTE: Separate forms are needed for each student registering.
• You have the option to save the completed form for your records by selecting download (the down arrow in the top right corner).
• To complete your registration:
  • You may email the completed form to kgoto@stmelgin.org and mail the $200 registration fee (per family) to:  St. Thomas More Catholic School, 1625 W. Highland Avenue, Elgin, Illinois 60123 
  • Or, print your completed form and return it to the school office with your $200 registration fee (per family).
