New students will  need to provide the documents below within 30 days of enrolling:

• Physical examination with immunizations: grades Preschool, K, and 6; also all new students

• Dental examination: grades K, 2, and 6; also all new students

• Vision examination by ophthalmologist or optometrist required for Kindergarten and new students entering a State of Illinois school for the first time

• Birth Certificate

• Baptism Certificate, if applicable
  
  
  In addition to aforementioned documents, Vision and Hearing Screening required for Preschool, 2nd and 8th grades. Service is provided by U-46 at STM School.
